Cannabis for Ulcerative Colitis and Crohn’s Disease treatment

“Chemicals found in cannabis could prove an effective treatment for the inflammatory bowel diseases Ulcerative Colitis and Crohn’s Disease, say scientists.

Laboratory tests have shown that two compounds found in the cannabis plant – the cannabinoids THC and cannabidiol – interact with the body’s system that controls gut function.

Crohn’s Disease and Ulcerative Colitis, which affect about one in every 250 people in Northern Europe, are caused by both genetic and environmental factors. The researchers believe that a genetic susceptibility coupled with other triggers, such as diet, stress or bacterial imbalance, leads to a defective immune response.

Dr Karen Wright, Peel Trust Lecturer in Biomedicine at Lancaster University, will be presenting her soon-to-be published work at The British Pharmacological Society’s Winter Meeting in London today (Thursday).

She said: “The lining of the intestines provides a barrier against the contents of the gut but in people with Crohn’s Disease this barrier leaks and bacteria can escape into the intestinal tissue leading to an inappropriate immune response.

“If we could find a way to restore barrier integrity in patients we may be able to curb the inflammatory immune response that causes these chronic conditions.”

Dr Wright, working with colleagues at the School of Graduate Entry Medicine and Health in Derby, has shown that cells that react to cannabinoid compounds play an important role in normal gut function as well as the immune system’s inflammatory response.

“The body produces its own cannabinoid molecules, called endocannabinoids, which we have shown increase the permeability of the epithelium during inflammation, implying that overproduction may be detrimental,” said Dr Wright.

“However, we were able to reverse this process using plant-derived cannabinoids, which appeared to allow the epithelial cells to form tighter bonds with each other and restore the membrane barrier.”

The research was carried out using cell cultures in a dish but, interestingly, when the team attempted to mimic the conditions of the gut by reducing the amount of oxygen in the cells’ environment, much lower concentrations of cannabinoid were needed to produce the same effect.

Dr Wright added: “What is also encouraging is that while THC has psychoactive properties and is responsible for the ‘high’ people experience when using cannabis, cannabidiol, which has also proved effective in restoring membrane integrity, does not possess such properties.”

The British Pharmacological Society (BPS) – the primary UK learned society concerned with research into drugs and the way they work – is hosting its annual Winter Meeting in London, attracting experts from across the world.

The three-day conference, running from 15 to 17 December 2009, will hear presentations on the latest pharmacological developments to tackle a range of conditions and diseases.”

Symptomatic therapy in multiple sclerosis: the role of cannabinoids in treating spasticity

“Anecdotal evidence suggests a beneficial effect of cannabis on spasticity as well as pain. Recently, randomized, double-blind, placebo-controlled studies have confirmed the clinical efficacy of cannabinoids for the treatment of spasticity in patients with MS. Based on these data, nabiximols (Sativex), a 1:1 mix of Δ-9-tetrahydrocannabinol and cannabidiol extracted from cloned Cannabis sativa chemovars, received approval for treating MS-related spasticity in various countries around the globe. In this article we review the current understanding of cannabinoid biology and the value of cannabinoids as a symptomatic treatment option addressing spasticity in patients with MS.

Based on individual case reports, the consumption of plant parts, specifically, the resin of the Cannabis sativa hemp plant, has, for years, been attributed to the capacity to reduce the symptoms of multiple sclerosis (MS), such as spasticity, neuropathic pain, tremor, and disturbed bladder function. As characterization of the endocannabinoid system and its role in the motor system and pain processing continue to advance, there is increasing evidence of a scientific basis for the postulated therapeutic effect of cannabis derivatives.

The oromucosal administration of THC and CBD in a 1:1 ratio has proven to be a well tolerated therapeutic option for treating spasticity in patients with MS who respond poorly to conventional antispastic drugs. Assessment of the efficacy is limited by the fact that spasticity as a symptom is very difficult to measure reliably, objectively, and validly. Current study data support the position that the beneficial effects of nabiximols on subjective and objective endpoints in a selected patient sample outweigh the adverse pharmaceutical effects. The effects of long-term nabiximols treatment on neuropsychological processes and the structure of the endocannabinoid system need to be further characterized.”

Dronabinol for supportive therapy in patients with malignant melanoma and liver metastases.

“Loss of appetite and nausea can reduce the quality of life of patients with malignant melanoma and liver metastases. Often established antiemetic drugs fail to bring relief. Tetrahydrocannabinol (THC, Marinol), which is the active agent of Indian hemp, has been used successfully in this situation for other malignant tumors.

PATIENTS AND METHODS:

We treated 7 patients with hematogenous metastatic melanoma and liver metastases suffering from extensive loss of appetite and nausea supportively with dronabinol (Marinol. All of these patients had previously received standard antiemetic therapy without adequate relief. Dronabinol is a synthetic Delta-tetrahydrocannabinol. The drug was administered in capsule form. We evaluated the palliative effects of dronabinol with a special patient evaluation form, which was filled out at the beginning of the therapy and again after 4 weeks.

RESULTS:

The majority of patients described a significant increase in appetite and decrease in nausea. These effects remained for some weeks, but then decreased as metastases progressed and the general condition worsened. All of the patients experienced slight to moderate dizziness, but it was not sufficiently troubling to cause interruption or termination of therapy.

CONCLUSION:

Loss of appetite and nausea due to liver metastases of malignant melanoma can be treated in individual cases supportively with Dronabinol.”

Cannabinoid-related agents in the treatment of anxiety disorders: current knowledge and future perspectives.

“Rich evidence has shown that cannabis products exert a broad gamut of effects on emotional regulation. The main psychoactive ingredient of hemp, Δ9-tetrahydrocannabinol (THC), and its synthetic cannabinoid analogs have been reported to either attenuate or exacerbate anxiety and fear-related behaviors in humans and experimental animals. The heterogeneity of cannabis-induced psychological outcomes reflects a complex network of molecular interactions between the key neurobiological substrates of anxiety and fear and the endogenous cannabinoid system, mainly consisting of the arachidonic acid derivatives anandamide and 2-arachidonoylglycerol (2-AG) and two receptors, respectively termed CB1 and CB2. The high degree of interindividual variability in the responses to cannabis is contributed by a wide spectrum of factors, including genetic and environmental determinants, as well as differences in the relative concentrations of THC and other alkaloids (such as cannabidiol) within the plant itself.

The present article reviews the currently available knowledge on the herbal, synthetic and endogenous cannabinoids with respect to the modulation of anxiety responses, and highlights the challenges that should be overcome to harness the therapeutic potential of some of these compounds, all the while limiting the side effects associated with cannabis consumption. Alzheimer’s: Marijuana as Effective Medicine

“Cannabis is getting much positive publicity and seems to be regaining its popularity of one hundred years ago when it was the most widely used drug for about 100 different diseases. There is a big toodoo nowadays about Alzheimers which has turned into almost an epidemic for old folks with some even in their 50’s. So far nobody seems to have put a handle on it and I won’t either.The latest information seems to be that there are senile plaques in the cerebral cortex and subcortical grey matter but whether these plaques are a cause or effect seems to be up for grabs.

I don’t know either. My Merck manual says that four million Americans have it, mostly those over 60 years old. It is a very expensive disease for nursing homes and nursing care probably at least 100 billion dollars per year.

As a Medical Marijuana doctor, I jumped into this fracas when I heard that a lady spouse of an Alzheimers patient went to a Medical Marijuana Advisory Committee meeting of The Oregon Medical Marijuana Program (OMMP) and demanded that her husband be given a permit. She was successful and the medical statistics from OMMP indicate that somewhat less than 50 people have Marijuana permits for this disease. It is called Alzheimers Rage.

The above really didn’t surprise me because there are several Central Nervous System (CNS) diseases for which Cannabis/Marijuana (C/MJ) works very well. These include ALS, Epilepsy, Multiple Sclerosis, Parkinsonism, PTSD and Traumatic Brain Injuries (TBIs). I don’t know if anyone has figured out why C/MJ works for these conditions but why ask if those suffering say that C/MJ works.

Tod Mikiyuria, the foremost scholar in this area, says that C/MJ works as a modulator which is a term of general meaning which possibly just means that it works.

The standard drugs are almost worthless, many seem to be Cholinergics related to Acetyl Choline, the standard Neuro transmitter nerve cell to cell and nerve cell to muscle. At any rate C/MJ seems to work better than any of the heavily advertised drugs.

Cannabis/Marijuana is simultaneously getting much positive publicity and seems to be regaining its popularity of one hundred years ago when it was the most widely used drug for about 100 different diseases.
